Liver tumors in children — hepatoblastoma and hepatocellular carcinoma (HCC) — are rare but serious cancers that require specialized care. Hepatoblastoma typically appears in children from infancy through about 10 years of age, and doctors often suspect it when a blood test shows very high levels of alpha-fetoprotein [e291-c1, e291-c2]. Premature babies have a higher chance of developing hepatoblastoma, usually in the first year or two of life . The main goal of treatment is to shrink the tumor with chemotherapy so surgeons can safely remove it . Doctors use a staging system called PRETEXT to describe how much of the liver is involved, which helps guide treatment decisions [e291-c11, e291-c12, e291-c13, e291-c14]. Most children receive chemotherapy first, and the tumor typically shrinks most after the first two rounds . Surgery to remove the tumor follows, and in some cases where the tumor involves too much of the liver, a liver transplant may be needed . Even when cancer spreads to the lungs, aggressive treatment including surgery to remove lung spots can lead to cure [e291-c17, e291-c27]. Overall, about 82% of hepatoblastoma patients achieve no evidence of disease, and survival continues to improve [e7221-c7, e11423-c2]. HCC is different — it doesn't respond well to chemotherapy, so surgery offers the only chance of cure, though outcomes are generally less favorable than hepatoblastoma [e10161-c2, e10161-c3, e10161-c4].
