Ultrasounds are not very good in terms of ruling out ovarian torsion
An ultrasound is a horrible test for torsion
In a patient with suspected torsion, you have to go to the operating room
When you find torsion in the operating room, you don't take the ovary out even if it's black; you leave it there in order to preserve fertility
After detorsion, you follow serially with ultrasounds to make sure that the ovary is staying healthy
Even if there's Doppler flow on ultrasound, doesn't mean that there's no torsion
Ovarian torsion could be intermittently torsing
There may be a part of the ovary that has blood flow while other parts do not
Even with Doppler flow present on ultrasound, you still need to go to the operating room for suspected ovarian torsion
